Medical Screening

Kayla does not have any ongoing medical concerns, though she is short-sighted and wears glasses or contacts to correct this.
Are you aware of any allergies you may have to foods, medications or environmental elements? If so, please list them below along with age of onset.
Kayla is Lactose Intolerant. While I am not certain of the exact age of onset, she has been this way for most of her life.
Do you take any other medications or drugs not prescribed to you, or participate in the consumption of alcohol or tobacco?
Kayla has been found in possession of Ritalin on three occasions. She is known to abuse it as a study aid, especially around the time of exams or coursework. I believe she has arranged via facebook to buy it from a pupil at her school, but I cannot prove this theory.

Environmental Screening
Where were you born and where did you grow up? What was it like there?
Kayla was born in Pohang, South Korea. She spent the first year of her life here, before moving first to Georgia, then on to Okinawa, back to Georgia, and finally to the UK. Until the final move, she had spent her entire life living on or around a variety of military bases. Moves were frequent, even when she was staying in one country, she never spent more than eighteen months in one place. The only constants in her life were her parents, and the cousin who lived with them. Her cousin's mother had walked out on her father some time previously, and the girl's father was killed in action. Kayla's father was the next of kin, and took the girl in.
She has told me little of what her life was like with her family, though from what I have been able to gather, life with her father was a strictly controlled environment. He appears to have held his family, Kayla; her mother, Haeun and her older cousin, Annie-May, to impossibly high standards, and reacted brutally when they were not reached. Physical and psychological abuse towards the three women appears to have been commonplace.
Please describe your relationships with your biological family, including parents, and siblings if applicable.
From what I understand, Kayla was very close with her mother, Sang, and even closer with her cousin. The three appear to have leaned on one another in order to survive her father's abuses. Anything from not getting straight As in class, to talking after lights out, breaking curfew, leaving a mess, and even entirely fictional reasons would earn them beatings, stress positions, and in later years, food or sleep deprivation, frequently a combination of several of these. I'm told he was always careful not to leave marks that would show while the girls were in school, apparently keen to avoid drawing attention to what was going on in the household.
Things did not stop there. As time went by, he became increasingly unhinged, and his treatment of his family grew worse. The three were too afraid to report him, apparently believing he'd kill them if they even tried.
Two years ago, that fear was realised. he beat his wife to death for reasons Kayla has never been able to explain, and hid her body in the freezer, claiming she had gone home to her parents. Kayla and Annie-May were sworn to secrecy, and threatened that there was room in the freezer for them too if they didn't 'start behaving themselves'. After this, Kayla states that she could almost never do anything right, and would be beaten to some degree most days. Sometimes it would be a slap to get her out of the way, or to demand she cook or clean faster, other times she'd be knocked unconscious for not achieving the right grades.
Things came to a head ten months after the death of Kayla's mother. She was working on a project for school, with a boy from her class. Her boyfriend in fact, not that her father knew. he also happened to be a Muslim, the son of a soldier on the base. Kayla's father returned home to find the two children in his living room, watching a movie they were apparently studying for school. On seeing them, and Kayla has assured me that at most the two were hugging, her father apparently flew into a worse rage than usual. He did not simply beat his daughter, he retrieved a knife from the kitchen and attacked both her and her boyfriend, Ali.
Ali did not survive the attack, and Kayla would likely have shared his fate if her cousin had not heard what was going on from upstairs and called the base police. Her father did not survive the ensuing firefight, and Kayla was hospitalised for two months following the attack. After her release, she moved to join her cousin, living with her surviving uncle in England.
It was only after his death that her mother's body was found.
How were/are you in school? How are your grades? Do you enjoy school?
Kayla has always been a straight A student, likely as she pushes herself far, far too hard to maintain her grades for fear of what will be done to her. She has told me that an A grade was never good enough for her father though, if she didn't get 100%, she got the belt. She once told me it was one strike for each mark she missed, regardless of how. This has lead to her abusing Ritalin as a study aid in a desperate attempt to maintain her grades, even now she is no-longer with her father.
Do you participate in any extracurricular activities or belong to any after-school clubs?
No. She states these would keep her away from her studies. I remain to be convinced that this is not an excuse to avoid social interaction
If you no longer reside with your biological family, please describe any previous homes as well as your current situation.
Following her removal from her father, Kayla was send to live with her uncle, his brother, as there were no other living relatives. This meant she'd be moving to England, as he was a retired US air force officer who'd settled down in the UK after retiring. Kayla interacts reasonably well with her cousins, Annie-May and her uncle's children, Tim and Jackie, but is terrified of criticism from her uncle and aunt. Though they have never done anything to warrant it, she will hide her test scores from them if they are anything less than 100%, and has expressed genuine fear over how they will respond to her GCSE module test results in March. She fears they will react the way her father did.
Do you have many/any friends? If so, please describe them.
Kayla has few, if any, friends. She has mentioned former friends in the states, but since the move, has not made much of an effort to socialise. She barely leaves her room.
Are you sexually active? Are you aware of any gender preference, or preferences in intimate practices, activities or situations?
I do not believe Kayla to be sexually active. She has mentioned kissing and cuddling with Ali, but nothing since.
Have you ever knowingly committed a crime? Were you caught and convicted? If convicted, what was your sentencing?
Only the possession of Ritalin mentioned earlier. On her first offence, she was given a caution. On her second, a fine. On her third, she received fifty hours of community service and was placed under a curfew for six months. She has four months of this curfew left, though I believe a change in the terms to allow her to attend Highgrove will not be a problem.

Clinical Screening
Have you ever been diagnosed with any psychiatric ailments by a medical or psychiatric professional? If yes, please explain below.
Kayla has been my patient for the past year. In that time, I have diagnosed her with Generalised Anxiety Disorder
Please describe any incidents associated with these ailments, such as antisocial behaviours, social or environmental difficulties, incidents of self harming, suicidal thoughts, suicide attempts, substance abuse, etc.
Kayla suffers from social difficulties, and almost refuses to interact with anyone outside of her family and myself. She displays anxiety in social situations, finds it very difficult to talk with her peers, and almost impossible to talk to adults she does not know. This may be a result of threats made by her father, or it may be another symptom of her GAD. She has, as I have mentioned, abused Ritalin as a study aid, particularly around exams, and has on one occasion been found self harming when her grades were not perfect. She burnt her legs with a cigarette lighter. I believe this is far from the only incident she has had, though it is the only one that has been discovered, and the only one she will admit to.
Have you ever seen a licensed psychiatric professional, psychologist, or counsellor for these ailments? If yes, please list doctor(s) or counsellor(s) below:
Kayla has been seeing me, Doctor Vaughan, since she moved in with her uncle a year ago. We have two sessions each week, one in her home, and one in my office.
Do you take any medications for any diagnosed psychiatric ailments? If yes, please list medication, dosage, and frequency of dosage below:
Thus far I have attempted to avoid the use of medication. I prescribed Fluoxetine briefly in August of last year, following her recorded self harming incident, but Kayla refused to take it.
Have you ever been admitted to a short or long-term psychiatric facility as a result of these ailments or associated incidents?
Kayla spent three days in short term care following her recorded self harming incident, returning to her family afterwards.
How do you feel these ailments affect your quality of life or your situation in life?
Her ailments prevent her from interacting with her peers, and to my knowledge, she only very rarely does things she actually enjoys instead of studying or sitting on her laptop. The closest she gets to willingly interacting with others is through constantly changing online pseudonyms, which she uses to play in pen and paper role playing games via instant messenger programs. She has told me that in these games, she never acts out of character,a nd enjoys pretending she is someone, or frequently something else.
If you could be cured of your ailments, what would your goals in life be?
I have discussed this with Kayla at length. She dreams of becoming an author, yet refuses to allow anyone to read her works.
How do you feel these goals would be best attained?
Kayla states that she does not know how best to go about this.
Why do you feel you are being referred to Highgrove, and by whom?
She feels she is being referred as she is 'not good enough' for her uncle and aunt. While her cousin is slowly adapting to the new situation and lack of constant pressure to succeed, Kayla is simply placing more and more pressure on herself. I am referring her to Highgrove, as I feel the change of scenery will be good for her, and the respite care her aunt and uncle are considering placing her in will not offer the controlled and safe environment I feel she needs to be in while she works to improve her social skills, regardless of weather or not she wishes to do so.

Hello, and thank you for meeting with me today. Our interview today will be recorded for review later. I'm going to ask you a series of questions, and I'd like for you to answer as fully as you are able. I'd like to start by asking you how you're feeling today.
Patient Observation: Kayla shrugs, her eyes on the ground. She looks nervous, even terrified.
Okay. And how are you feeling about this opportunity to join our family at Highgrove?
I...already have a family.
Patient observation: Speaks in a quiet voice, with eyes closed and arms crossed protectively.
If you could alter one event in your life, what event do you think you would change or remove, and why?
I...don't know.
Patient observation: places hands in lap, obsessivley tugs at blouse, seems to be trying to remove creases, though none can be seen.
If you had to describe yourself in only three words, what words would you choose and why?
Stupid, useless, gook.
Patient observation: Extreme hesitation. This answer takes several minutes, and is accompanied by some trembling.
How do you feel currently about your life so far, and anything you may have done?
Not good enough
Patient observation: Voice barely audible, eyes still closed, hands now in lap.
What would you do if you found a stranger being attacked by another stranger, and why?
Patient observation: Kayla does not answer this question, she simply whimpers. When it is repeated, she shakes her head.
What would you do if you were being threatened by a stranger, and why?
Patient observation: Again, Kayla refuses to answer, though she has opened her eyes now, and is staring at her knees.
Have you ever deliberately harmed yourself, another, or animals in the past? If so, why do you think that was?
Patient observation: Again, she does not respond, though her hands move, covering the locations of suspected self harm.
And lastly, what do you hope to find at Highgrove, and how do you think this will help you to learn and grow?
